A chart is simply a list of client related documents, or "Chart Items".
Examples of chart items are assessments, service plans, encounters, and
discharge summaries. Files related to clients can also be uploaded and
listed like any other chart item in the chart. All the clinical
information for a client is available in the chart, and ONLY available
under the chart menu item or chart item report. Therefore users of the
system that should not have access to any clinical information should
not be given access to the Charts menu item or Chart Item report.
